Indications

There are many different indications for treatment with sedation.

  • In adult patients with phobia, anxiety or panic of the dentists when performing dental treatments.
  • In patients that require treatments in one single session for their better comfort.
  • In special situations: patients with extensive pathologies, bad behavior, systemic pathologies, specific syndrome and traumatic injures (e.g. patients with Parkinson disease, Alzheimer’s disease, Down’s syndrome, etc).
  • In patients with little time available or those who arrived from other provinces, since this allows us to dramatically decrease the number of sessions needed for any dental treatment especially for implantology.
  • In children when it is not possible to perform a conventional dental treatment.

Treatment with sedation

One of the most common treatments in which we apply conscious sedation techniques refers to oral surgery and when it comes to the implant collocation to rehabilitate the mastication (chewing) function.

 

They are activities which by their duration require deep and intense anesthesia of all the tissues, since practices of high precision have to be applied and it is important that the patient was in the state of maximum relaxation, and thus any stressful situation that might affect the success of the treatment can be avoided.

During all the surgery process the patient will be in a state in which he/she will feel no pain, he/she will not see the syringe needle, nor will he/she hear any noise, which also can help avoid many psychological traumas that could influence the future surgery treatments.

Besides the conscious sedation allows to ensure the maximum safety throughout the treatment since at all times the patient will be monitored and controlled by an anesthesiologist, which is known in the ASA (American Society of Anesthesiologists) as a Monitored Anesthesia Care (CAM), taking control of the vital signs of the patient and providing him necessary medical care and also the subsequent management after the treatment is over.

First visit: Communicating with the patient and drawing a treatment plan

Treatment starts with the first contact. When a person that has a dental phobia comes to visit us for the first time we are well aware of the great endeavor he/she has had to make, so from the very first moment we will treat him with care and understanding.

Communication is key in our program. It is an essential key factor both for our assisting and medical staff.

You will have all the time you need to explain to us your concerns and worries of what happens. You will be able to make all the questions you want both about the steps to follow in each moment and the treatment to be performed. You will be in a cozy and warm atmosphere for your major convenience. You will receive a caring and sincere treatment from all our staff, all assisting, medical and administrative staff, who after having made a complete study of your case will draw you an individualized plan of treatment.

We will explain you all the options of the possible treatment and you will be assured that all of them are completely safe and harmless.
We will not exceed your limits nor will we advance faster than it is necessary, since every step you advance and your expectations meet will make you feel better and keep on recovering your confidence.

Second visit: Treatment without pain 

The day of the intervention you will see neither dental instruments nor syringe needles, you will not hear any noises that might make you feel uncomfortable.

We use the latest advances and techniques in Sedación so that you were sure you will not notice  Any Pain either before or during the treatment. We want your fear of dental specialist to be converted into a positive experience and you to be an active participant in this treatment.

All the treatment performed under conscious sedation will be monitored by Dr. David Martínez, physician anesthesiologist specialized in dental treatments. We do not work with general anesthesia, so the hospitalization is not needed nor is the fasting 24 hours before the intervention; because this could cause you more stress, anxiety or increase of the panic toward a dentist and would not help you overcome the problem you have.

After the intervention you will have a feeling that hardly 15 minutes have passed, however in fact we will have been working more than 3 hours. You will have a scarce memory of what happened and you will be able to return back home always accompanied and following the instructions you will be given.

In one single intervention we can improve your quality of life and also we will return you the confidence so that you could smile again.

Third visit: Revision, prosthesis collocation (if needed) and follow-up of the treatment.

After one week we will examine the patient to check if the treatment has been satisfactory and indicate the hygiene norms he will have to follow.

From then on in most cases a patient comes to visit the doctor one month later for management and checkup if the level of the patient’s anxiety or dental fear has reduced significantly; since the patient himself participated in overcoming of this problem.

In case we have placed implants or temporary crowns on a patient, in this session we will collocate a permanent one, for which there is no need to perform any type of the treatment that would require anesthesia since it is just a routine work. Also in this session we check if the color and shape of the teeth are those the clients always wanted to have.

Relaxation techniques to overcome the dental fear

The great majority of patients that visit a dental room demonstrate at least some degree of discomfort, tension or fear of the dentist. This is why when the patient is taken to the SEDENTIST program, a caring and friendly treatment helps a lot.

Our duty is to show the patient how he/she can relax with the means that the modern dentistry provides us.

A relaxed patient is easier to deal with and is more open for alternatives of his treatment comparing to the patient blocked by his fear of the dentists.

Among the techniques used in dentistry and those that come from other fields there are:

    • Aromatherapy

 

The dental clinic environment itself can induce the feeling of fear. The typical smell of a dental room is one of the causes of such fear.

Aromatherapy uses the essential oils of plants as a therapeutic method. It tries to awaken the innate healing processes of each person.

It is important to make sure that pure, natural undiluted oils are treated, since the chemical aromas are lacking in the plant properties.

    • Virtual relaxation glasses

 

It is an instrument of distraction and relaxation that helps the patient avoid visualizing the instruments and objects that are inside of the dental room, decreasing panic of the dentists. Changing the focus of attention is what makes it beneficial both for a patient and dentist.

These virtual glasses of the new generation are smaller and more comfortable so that the dentist can do his job. The patient will have the feeling of watching an image of 1,5 meters diameter to 3 meters of distance with VGA resolution. The volume level is adjustable, which ensures the proper communication with the patient.

If you see it like that, the only thing a patient would be missing to believe that he/she is at the cinema is the popcorn. But it pollutes the teeth which is something we try to avoid.

    • STA Computer assisted anesthesia system

Among the situations that cause the most anxiety and dental fear; there is a visualization of the syringe and the feeling of the anesthesia entering the area to be treated.

Treatment of the children with dental fear

A preliminary visit should be arranged for a child before the treatment is started, in order to familiarize him more with the environment of the dental clinic.

Our staff is familiar with dental phobia. We must tell the truth to a child but without emotional exaggerations.
It is a very important function of the pediatric dentist. It has the same function as a pediatrician and a school professor so that a child learns how to control his behavior and anxiety in a dentist’s room. The technique “tell me – show me – do it”, as well as the exposition and introduction of the dental instruments to a child, as well as gingering a child up when he follows the instructions of the dentist.

Items that may increase the stress of anxiety of a child (such as fixators or blocks to keep the mouth of the child open) should be eliminated.

Duration and time of the visit: For the children of the preschool age the doctor visit should be short (less than 30 minutes) till a child gets familiarized with the instruments. For preschoolers it is also better to arrange the doctor visits in the morning since at this time of the day children are not yet tired, there are less delays in visits (less time to wait in a waiting room) and the specialist is more fresh.

It would be better if the parents do not enter inside the dentist room with a child from 3.5 years age. The fact that a parent is staying inside can distract the dentist, and the child will try to attract attention and make parents feel compassion, who will justify that a child wants to postpone the dental treatment.
We should avoid a famous saying: “Did he hurt you?” Or also: “Do not worry, doctor, I will talk to my son”.

Although most of the parents affirm to explain their children about the treatment, the pediatric dentist should explain all about the treatment and not parents.

Relaxation and distraction techniques /Audioanalgesia: 
5-10 minutes are enough before starting the treatment. It consists of applying music or stories through headphones of a reduced size. Depending on the age of the child there will be children’s songs, fairy tales, adventure stories, etc. It is often effective to alleviate tension and to reduce the fear and defensive reactions of a child, which always makes the dentist’s work easier.

Psycopharmaceuticals: (diazepam, etc)
They are used when the behavior and control of a child during the treatment are difficult and cooperation is complicated. A child has the following symptoms: scared of the sight of the instruments, has tension in the legs or intents lift them, cries and doesn’t talk nor establish communication with the pediatric dentist.
Psycopharmaceuticals allow the treatment but do not make the communication with the dentist better.

Sedation: It is necessary when it is not possible to perform the treatment with the pharmaceuticals, or also in case the treatment is more aggressive.

Preventive measures: It is the best treatment in order to avoid the child had fear of visiting a dental clinic. For this reason it is necessary to explain the methods of the oral hygiene from the early childhood (at the beginning of the eruption of the primary teeth), and parents are the best example of practicing these methods.
Children must have a balanced diet, and both parents and school should give appropriate dietary advice, advices on the use of fluoride/tooth wash and about the techniques and frequency of brushing teeth after each meal.
In order to prevent caries both of the primary teeth and permanent teeth, regular visits to the pediatric dentist should be made in order to check the condition of the gums since the early diagnosis is very important.
We should discard the idea that since this tooth is “primary” there is no importance if it has caries, because the function of such teeth is very important during the process of the replacement of teeth both by saving space and by guiding the eruption of the permanent teeth.
Finally it worth emphasizing that most of the phobias of the dentist of adult patients are caused by traumatic experiences in childhood and adolescence. This is why we insist in the importance of the oral health in this stage of life.
